I need a 6 panel trifold brochure (8.5"x11"), for my spine and pain management practice. It must be complete and ready to print. Keeran Kumar M.D. Interventional Spine and Pain Management. Conveying the necessary information in a professional looking brochure is the goal. I prefer simple elegant design but i am also very flexible. I need it complete and ready to print with information on each panel. I have a logo, a personal photo and the wording for each panel to upload. I will also submit my business cards for viewing. Additionally, i need a front cover design and a back cover design for a Folder 8.5"x11" (full page) that will contain the brochure and other marketing materials. 1. Referrring Doctors 2. Prospective Patients I will be taking this brochure to other doctors offices for referrals; to let them know who I am and what I do. I will also have copies in my office for patients to see what services are offered. I need a professional looking brochure. |

A total of 6 panels in the trifold brochure. 3 panels each side. I will upload the wording for each panel. SIDE 1 Right Panel (front cover): to have practice title, logo, phone/fax, address and Keeran Kumar M.D. SIDE 1 Middle Panel (back cover): Pain Conditions Treated SIDE 1 Left Panel: Pain Procedures Performed. SIDE 2 Left Panel: Practice Information SIDE 2 Middle Panel: Personal photo and Biography. SIDE 2 Right Panel: Practice Philosophy. I will upload my logo, a personal photo, and the wording I need on each panel. I am very flexible with design as long as it has the information i need. if you the designer has other stock photos or images to use, please do so. For the Folder design, i need; Front Cover with Keeran Kumar, M.D. logo, business name, and fax and phone numbers, plus any design elements. Back Cover i need the logo with practice information, address etc plus any extra design elements. |
