I am an International Retina Surgeon. I am developing dedicated eye surgery centers in Saratoga Springs, New York, in Amman, Jordan and in Dubai, UAE. In my private practice in Saratoga Springs, New York, I have found that patients are scared of losing their vision and are looking for clear answers they can understand. I wrote this book because diabetic patients make up a large percentage of my practice and is the leading cause of blindness in the US. May cases of blindness could have been prevented with eye exams. I require a front and back cover (5-1/2 inches by 8-1/2 inches) plus a narrow spine for my 80-page SOFT-COVER book. The title is “Diabetes and Your Eye”. The Sub-Title is “7 Steps to Prevention & Control of Diabetic Eye Conditions”. The Author is Amjad M. Hammad, MD, MBA There must be a starburst or some other noticeable announcement on the back cover “More Free Resources at www.SaratogaRetina.com” You will need to leave space on the back cover to insert an ISBN. You will insert my photo (on the back cover next to the author bio) and you can choose which one you like, how big it is and where to place it. Author Bio: Dr. Amjad Hammad is the owner of Saratoga Vitreo-Retinal Ophthalmology, CEO of The New York Eye Surgical Center and Clinical Assistant Professor of Ophthalmology at Albany Medical College. He has made his home in beautiful Saratoga Springs, NY with his wife Terry. The cover needs to be serious and give the impression it was written for someone living with diabetes. I have included images of vision loss that a person may be experiencing with diabetic retinopathy. The font needs to be large print, as our target audience has vision problems. The price is $16.95 and the words “First Printing” need to be on the back cover. The spine is the standard stuff on a spine. Use this copy for the back cover. Back Cover Copy: Diabetes is the leading cause of blindness and nearly 30% of those diagnosed with diabetes will have diabetic retinopathy. My goal in writing this book is to help you understand your condition, find answers and more importantly avoid unnecessary vision loss. I am so proud of all my patients who have worked so hard to control and mange their diabetes for better health. This book allows me to help others gain better control over eye health and know the diabetic eye diseases are preventable and treatable. |
