This custom folder will be used to hold the results of a patients test and will be sent home with our patient. This folder must really accurately reflect who we are and also act as a point of education for the patient as well. We are upscale with an emphasis on patient service, our core values, what a patient can expect from us, and what we expect from our patients. It should also educate the patient on a few key points regarding the nervous system which will be uploaded for you. |

Folder must utilize both front& back covers and inside as well. Design must include our tag, mission,core values, description of our ideal practice member, Defination of a vertebral subluxation, area for the doctor to list pt diagnosis, area for doctor to list patient primary goal, definations of the 23 dimensions of stress,and nervous system diagram with possible symptoms. |
