  I will need to be able to change names etc on them for other workers.
Attached you will find a form that we use regularly (see the comment for the file).
I would also need the stationary designed in a way that is easy to use for word processing in MS Word, such as a template or in a word document that I can type on, besides in any way that may be printed.

 
  I am guessing to use the secondary logo for the back but this is not required. An idea that I had but am unsure how it would look is to make that logo larger and then to place the words inside the lower grey area.
I may choose a full color design with pictures besides the more traditional one which I prefer in order to give the sales force a choice.

This is a form which we use regularly. We primarily use the excel version here: http://lowerwatt.com/wp-content/uploads/2012/03/Lower-Watt-LOA-Form-unprotected.xlsx The form gets minor edits from time to time and I would prefer to have it designed as a header and footer or a template.
