    These are some icon sets I found on the internet.

    Here, I like the third icon from left to right bottom row.

    Here, first set, last row, second icon. I like this one a lot. It would be nice to have instead of the pencil a cursor. And either the caduceus or the red cross.

    OK, let's see if I can explain this better.

    So basically, instead of writing in the paper chart, the doctor will be a using a computer to document the patient's visit. For example, let's say you go to the doctor. After he (or her) examines you and decides the course of treatment, he has to document it in the paper chart. Right?

    So now imagine that instead of using paper, the doctor uses a computer. And instead of having to write every single thing, some parts are already written and he just has to complete the note. Just like using a pre-filled paper form and checking what applies (check mark).

    So that's what Mediplates are: pre-filled, computerized forms to help the doctor document the medical note. It's going from paper chart to electronic medical record.
