I need business cards and letterhead. There are two doctors in the office, please put both names on the business cards. They do not need separate cards. Please put the phone number for After Hours underneath the fax number. That number is 830-997-3851. I would like the front of the card to be colorful. But I would like the back of the card to be predominately white with color accents so that it is easy to read once the appointment information is written on it. Please try to incorporate the logo on the back as well.

Appointment card on the back. Line for the name. Under the line "Has an appointment on" then checkboxes for days of the week M,T,W,Th,F. Then another line for the Date and Time of the appointment and another line for Notes. Underneath everything the sentence: If you are unable to keep this appointment, please notify us within 24 hours.
